Simply fax over the completed Patient Referral Form along with the patients current demographics. We’ll do all the rest!
When should my referring patient expect to be contacted by your office?
Our goal is to contact referrals within 1-2 business day of receiving their referral form.
How long will it take before a patient is seen?
We usually see routine patients within 5-7 business days. Urgent needs will be addressed promptly and on a case-by-case basis.
Will your office keep me appraised of treatments and progress?
Yes, a complete consultation is dictated and faxed to your office. Further treatments, significant changes in medication or recommendations are always done by fax, and if necessary by phone.
Does your office accept Medicare?
We accept Medicare assignment and offer Medicare patients all covered services. Take a look above to see a full list of accepted insurance plans.
Does your office accept Workman’s Comp?
No, we do not accept Workman’s Comp at this time.
Does Dr. Lewis work with chiropractic patients?
Yes, Dr. Lewis enjoys helping all chiropractic patients in support of their current chiropractic treatments.
Will you see patients who may have a medication problem?
Yes, we can help reduce or “wean off” patients who have developed a medication problem. We do this in a very gentle, non-confrontational manner utilizing recognized methodologies.
Will you provide narcotics for patients who need them?
Yes, there is a sub-set of patients who must have opioid analgesic to remain functional and control their pain. While it is always our goal to reduce opioid use, we have patients in our practice who we manage with the judicious titration of narcotic analgesics. These patients sign a narcotic contract, are monitored carefully in clinic, and all prescriptions are documented.
Do you use psychology, rehab, and other modalities?
Yes, we refer to several clinical psychologists. We utilize PT and REHAB services when appropriate, and will notify you when indicated.
What hospitals is Dr. Lewis affiliated with?
Baylor Regional Hospital at Plano, Baylor Regional Hospital at Frisco, Presbyterian Hospital of Plano, Medical Center at Craig Ranch, Cornerstone Surgery Center and Windhaven Medical Plaza.
How can I learn more about Dr. Lewis and Lewis Pain?
There is no better way than to meet Dr. Lewis yourself. How about lunch? Dr. Lewis would like to meet you at a restaurant or bring lunch to your office – whichever you prefer. How do I get more referral forms or business cards?
There are two options! The quickest way is to download a referral form from this page. The second option is to choose "I need referral forms/business cards" in the form to the left. Make sure to fill out the physician name and address. Our marketing director will be happy to personally deliver or mail these to your office. Please allow 1-3 business days for delivery.
