Physician Referral Process:

Call 214-618-9600 or fax a completed Patient Referral Form (or order) with current demographics, insurance information, recent physician notes, and radiology reports (X-rays, MRIs, CT scans…) to 214-618-7997. We will verify insurance benefits and contact the patient to schedule an appointment at one of our convenient locations. A report with Dr. Lewis’ findings will be faxed to the referring physician’s office.

Billing Information:

Patients are expected to pay appropriate co-pay, co-insurance, and deductible amounts prior to appointments and procedures. Please note that any remaining amounts will be billed to the patient. Since several entities (Lewis Pain Division, hospital, anesthesia) are involved, patients may receive several different bills for one procedure.

Pre-Procedure Instructions:

If you are taking a blood thinner such as COUDAMIN, WARFARIN, PLAVIX, LOVENOX, or HEPARIN, please notify us prior to your procedure. This has to be approved by your physician. Approvals for you to be off the medication need to be faxed to 214-618-7997.

You must arrange for a driver (not to include a taxi, bus or any public transportation) after your procedure.

Do not eat or drink anything 6-8 hours prior to your procedure. NO WATER, GUM, MINTS, NOTHING!

If you routinely take medications in the morning for high blood pressure, cardiac conditions, seisures, reflux, or asthma, you may take them with as little water as possible.

If you are wearing a Duragesic (Fentanyl) Patch and having sedation, it must be taken off 12 hours prior to the procedure.
